This glossary breaks down the key terms you need to know, from common drugs like cocaine and heroin to critical concepts like dependence, tolerance, and detoxification. These terms form the foundation for understanding how substances affect the body and mind, and how recovery begins.

Common drugs: cocaine and heroin
Cocaine is an addictive stimulant made from the leaves of the coca plant, which is native to South America. While cocaine can be used as a local anesthetic for certain eye, ear, nose, and throat surgeries, most cocaine is illegally produced and consumed. The drug produces intense feelings of euphoria, increased energy, and heightened mental alertness, but these effects are followed by serious negative consequences including increased irritability, restlessness, and panic attacks.
Heroin is a highly addictive opioid derived from morphine, which comes from the seed pods of the opium poppy plant. Unlike cocaine’s stimulating effects, heroin causes euphoria followed by feelings of relaxation and sedation. The drug is particularly dangerous because it significantly slows breathing, and high doses can lead to life-threatening respiratory depression. Both drugs carry severe risks including overdose, infection, and the development of substance use disorders.
Understanding dependence: physical vs. psychic
Dependence on substances manifests in two distinct but interconnected ways. Physical dependence occurs when prolonged use of a drug creates an altered physiologic state, and the body requires the substance to function normally. When someone with physical dependence stops taking the drug, they experience withdrawal symptoms as the body struggles to readjust. These symptoms can range from mild discomfort to severe, life-threatening conditions depending on the substance.
Psychological dependence, also called psychic dependence, refers to the emotional and mental aspects of addiction. This involves intense cravings for the drug’s euphoric effects and the perceived need to continue using despite negative consequences. Someone with psychological dependence might obsess over obtaining the substance, experience anxiety when they can’t use it, and continue seeking the drug even when they recognize the harm it causes.
The distinction between physical and psychological dependence matters, but they often occur together. A person physically dependent on alcohol will experience tremors, nausea, and potentially dangerous seizures when they stop drinking. Simultaneously, they may experience intense psychological cravings and anxiety about functioning without the substance. Both forms of dependence require professional treatment for safe and effective recovery.
How dependence develops
With repeated substance use, the brain adapts to having the drug present. These neurological changes affect how the brain’s reward system functions, creating both physical and psychological need for the substance. What starts as occasional use can progress to regular use, then to dependence as the body and mind become accustomed to the drug’s presence.
Tolerance and withdrawal
Tolerance develops when the body becomes used to a substance, and the same amount no longer produces the same effects. Someone who initially felt intoxicated after one drink might need three or four drinks to achieve the same feeling after developing tolerance. This happens because the body adapts to the substance’s presence, requiring increasingly larger doses to produce the desired effect.
Tolerance is particularly dangerous because it drives escalating substance use. As people need more of a drug to feel its effects, they consume larger quantities, increasing their risk of overdose, health complications, and developing severe dependence. For instance, someone with heroin tolerance who stops using and then relapses faces heightened overdose risk because their body can no longer tolerate the doses they previously used.
The withdrawal experience
Withdrawal encompasses the physical and psychological symptoms that emerge when someone reduces or stops using a substance after developing dependence. These symptoms vary dramatically based on the specific drug, duration of use, and individual factors. Alcohol and benzodiazepine withdrawal can be life-threatening, causing seizures and severe dehydration. Opioid withdrawal, while rarely fatal, involves intense muscle pain, nausea, and severe discomfort.
Common withdrawal symptoms include anxiety, depression, insomnia, sweating, tremors, and intense cravings. The severity of withdrawal explains why many people struggle to quit substances on their own. The body has adapted to function with the drug present, and removing it creates a state of physiological distress. This makes professional medical supervision during withdrawal essential for safety and comfort.
The process of detoxification
Detoxification is a medical intervention that safely manages an individual through acute withdrawal. Rather than stopping substance use abruptly, detoxification involves carefully managing the withdrawal process under professional supervision. This gradual approach helps minimize withdrawal symptoms and reduces medical risks.
The detoxification process involves three essential components. First, evaluation occurs where healthcare providers test for substances in the bloodstream, assess medical and psychological conditions, and determine appropriate treatment levels. Second, stabilization guides the patient through withdrawal, often using medications to ease symptoms while providing emotional support. Third, preparing the patient for ongoing treatment emphasizes that detoxification alone is not sufficient for recovery.
Methods of detoxification
Detoxification can involve various approaches depending on the substance and individual needs. Medical detoxification uses medications to manage withdrawal symptoms and prevent complications. For alcohol withdrawal, medications prevent seizures. For opioid withdrawal, medications like buprenorphine reduce cravings and ease discomfort. Some programs use tapering, gradually decreasing doses of the substance or a similar medication to ease the transition.
Alternative methods like acupuncture have shown promise in supporting detoxification, though they typically complement rather than replace medical approaches. The key is that detoxification occurs under professional supervision in an appropriate setting, whether inpatient, residential, or outpatient, based on the severity of dependence and individual circumstances.
Beyond detoxification
It’s critical to understand that detoxification is not substance abuse treatment itself. While detoxification addresses physical dependence and manages acute withdrawal, it doesn’t resolve the psychological, social, and behavioral problems underlying substance abuse. Successful recovery requires comprehensive treatment following detoxification, including counseling, behavioral therapy, and ongoing support to address the root causes of addiction and develop coping strategies for long-term sobriety.
