Alcohol: the socially accepted depressant
Alcohol is one of the most widely used psychoactive substances globally, yet many people underestimate its dangers. When consumed, alcohol initially produces feelings of well-being and relaxation, but it actually functions as a central nervous system depressant that slows brain activity and impairs coordination and judgment.
Long-term alcohol abuse leads to severe health consequences. Chronic drinking can result in liver damage, brain damage, and significant mental impairment. The liver, which processes alcohol, becomes scarred and develops cirrhosis. Brain tissue deteriorates, affecting memory, decision-making, and emotional regulation.
Alcohol withdrawal and delirium tremens
Withdrawal from alcohol can be particularly dangerous. Symptoms typically include anxiety, sleeplessness, tremors, and in severe cases, a life-threatening condition called Delirium Tremens. Delirium Tremens usually develops 48 to 72 hours after the last drink and involves severe confusion, hallucinations, high fever, rapid heart rate, and profuse sweating. Without treatment, the mortality rate can reach 15%, but with proper medical care, survival rates improve to about 95%.
Narcotic analgesics: from pain relief to addiction
Narcotic analgesics, also known as opioids, include naturally occurring substances like opium and morphine, as well as semi-synthetic drugs like heroin. These substances can be taken orally, injected, or smoked. They work by activating opioid receptors in the brain, producing euphoria, pain relief, and a sense of mental relaxation.
The dangers of opioid abuse are substantial. Users may experience acute anxiety, develop nasal ulcers from snorting, and suffer brain damage from chronic use. Long-term injection drug use carries additional risks including HIV infection, hepatitis, and damage to veins and heart valves.
Opioid withdrawal and tolerance
Opioid withdrawal resembles a severe flu, with symptoms including muscle and bone pain, nausea, vomiting, diarrhea, sweating, anxiety, and dilated pupils. The withdrawal typically begins 8 to 24 hours after the last dose for short-acting opioids like heroin, and can last 4 to 10 days. While generally not life-threatening, opioid withdrawal is extremely uncomfortable and often drives people back to drug use.
Tolerance to opioids develops quickly. Users need increasingly larger doses to achieve the same effects, which significantly increases overdose risk. Most opioid overdose deaths occur in people who have recently detoxed, as their tolerance has decreased.
Stimulants: the dangerous rush
Stimulants like cocaine are substances that speed up central nervous system activity. Cocaine produces intense euphoria, increased energy, mental alertness, and hypersensitivity to sight, sound, and touch. Users may feel talkative and confident, but these effects are short-lived.
Cocaine works by blocking the removal of dopamine from brain synapses, creating an artificial flood of pleasure signals. This mechanism makes it highly addictive. Long-term use can cause heart rhythm disturbances, heart attacks, bleeding in the brain, and severe psychological problems.
Stimulant withdrawal
Cocaine withdrawal primarily involves psychological symptoms rather than physical danger. Users experience severe depression, ongoing tiredness, insomnia or excessive sleeping, intense irritability, increased appetite, and difficulty concentrating. These symptoms can be so distressing that they drive users back to cocaine use, making abstinence challenging.
Depressants: the sedative danger
Barbiturates and benzodiazepines are central nervous system depressants that slow down brain activity. These drugs enhance the activity of GABA, an inhibitory neurotransmitter, producing calming and sedating effects. While useful medically for treating anxiety and sleep disorders, they carry significant abuse potential.
Depressant withdrawal risks
Withdrawal from barbiturates is particularly dangerous. While benzodiazepine withdrawal can be problematic, withdrawal from prolonged barbiturate use can have life-threatening complications. Symptoms include nervousness, tremors, agitation, seizures, delirium, and in severe cases, death. Both barbiturates and benzodiazepines require medical supervision during withdrawal due to the risk of fatal seizures.
Hallucinogens: altering perception
Hallucinogens like LSD cause dramatic changes in perception, mood, and thought processes. Users experience visual and auditory hallucinations, altered sense of time, and intense emotional experiences that can range from euphoric to terrifying. These drugs primarily affect serotonin receptors in the brain, particularly the 5-HT2A receptor.
The risks of hallucinogen use include triggering persistent psychosis or long-term mental health problems. Some users experience flashbacks weeks, months, or even years after use, where visual distortions suddenly recur. While classical hallucinogens like LSD don’t cause physical withdrawal symptoms or physical dependence, they can nevertheless have serious negative long-term psychological impacts.
Cannabis: beyond the high
Cannabis products like marijuana and ganja produce their effects through THC (tetrahydrocannabinol), which interacts with cannabinoid receptors in the brain. Users experience euphoria, relaxation, altered perception of time, and heightened sensory experiences. However, cannabis use can also result in paranoia, anxiety, fatigue, and impaired memory and concentration.
Cannabis withdrawal, while milder than other substances, still causes uncomfortable symptoms including sleep problems, irritability, decreased appetite, and anxiety. Tolerance to cannabis develops with regular use, though at a lower rate compared to opioids or stimulants.
Drug classifications: understanding the categories
Drugs are classified in two main ways: by their effect on the central nervous system and by their origin.
Classification by effect
Narcotic analgesics: Substances like heroin and morphine that relieve pain and produce euphoria by activating opioid receptors.
Stimulants: Drugs like cocaine and amphetamines that increase central nervous system activity and energy levels.
Depressants: Substances like barbiturates and alcohol that slow down brain activity and produce sedation.
Hallucinogens: Drugs like LSD that alter perception, mood, and cognitive processes through serotonin receptor activation.
Classification by origin
Natural drugs: Substances extracted directly from plants, such as opium from poppies and THC from cannabis.
Semi-synthetic drugs: Chemically modified natural substances, like heroin (derived from morphine).
Synthetic drugs: Completely laboratory-created substances like barbiturates and many prescription medications.
Designer drugs: Synthetic substances chemically engineered to mimic other drugs while evading legal restrictions, such as MDMA (Ecstasy). These often have unpredictable and dangerous effects due to their high addictive potential and unknown long-term consequences.
The common thread: tolerance and dependence
When drugs are used regularly, the body adapts by adjusting neurotransmitter systems to maintain balance. This adaptation leads to tolerance, where larger doses are needed to achieve the same effect. Physical dependence develops when the body requires the drug to function normally, and stopping use triggers withdrawal symptoms.
The severity of withdrawal varies dramatically by substance. Alcohol, benzodiazepines, and barbiturates can cause life-threatening withdrawal requiring medical supervision. Opioid withdrawal, while not typically fatal, causes severe discomfort that drives relapse. Stimulant and hallucinogen withdrawal primarily involves psychological symptoms.
