Partner infection status determines your baseline risk
The most fundamental factor in HIV transmission during sex is whether your partner has HIV. If your partner does not have HIV, there is zero risk of HIV transmission from sexual contact. However, if your partner is living with HIV, the transmission risk depends heavily on their treatment status and viral load.
Research has established that people with HIV who maintain an undetectable viral load through treatment have zero risk of sexually transmitting the virus. This concept, known as Undetectable equals Untransmittable (U=U), has been confirmed through extensive studies following thousands of couples over many years. When someone living with HIV takes antiretroviral therapy as prescribed and achieves viral suppression below 200 copies per milliliter of blood, sexual transmission does not occur.
Conversely, when a person with HIV is not on treatment or has a detectable viral load, transmission becomes possible. The World Health Organization states that those with viral loads above 1,000 copies per milliliter face increased risk of transmitting HIV to their sexual partners. During the acute phase of HIV infection, which occurs within 2 to 4 weeks after someone contracts the virus, viral load is at its highest and transmission risk increases significantly.
Type of sexual contact creates different risk levels
Not all sexual activities carry the same risk for HIV transmission. Receptive anal intercourse poses the highest risk, with approximately 138 infections per 10,000 exposures, making it substantially riskier than other sexual activities. This elevated risk occurs because the rectal lining is thin and can tear easily during intercourse, creating direct pathways for the virus to enter the bloodstream.
Vaginal sex carries moderate risk for both partners. HIV can enter through the tissue lining the vagina and cervix, or through the opening at the tip of the penis. The presence of vaginal fluid and blood during intercourse can facilitate transmission through these vulnerable tissues.
Oral sex presents considerably lower risk compared to vaginal or anal intercourse. However, certain factors can increase oral transmission risk, including ejaculation in the mouth when oral ulcers, bleeding gums, or genital sores are present. The presence of other sexually transmitted infections during oral sex also elevates the risk of HIV transmission.
Why penetrative sex carries higher risk
Penetrative sexual acts create opportunities for direct contact between infectious fluids and mucous membranes or damaged tissue. The rectum, vagina, and penis all contain mucous membranes where HIV can enter the body. During penetrative sex, microscopic tears or abrasions can occur, further increasing the risk of transmission. Activities that cause additional trauma to these tissues, such as rough sex or sex without adequate lubrication, compound the risk.
Viral load and STDs significantly amplify transmission risk
Among people living with HIV who are not virally suppressed, the amount of virus in their blood directly correlates with transmission risk. Research shows that transmission risk increases substantially when viral load exceeds 1,500 copies per milliliter. Higher viral loads mean more virus particles are present in bodily fluids, making transmission more likely during sexual contact.
The presence of sexually transmitted infections dramatically increases HIV transmission risk for both the person with HIV and their partner. STIs can cause sores or breaks in the skin, making it easier for HIV to enter the body. Inflammatory STIs like gonorrhea and chlamydia increase the concentration of HIV in genital secretions.
Studies demonstrate the powerful effect of co-infections on transmission. Research indicates that STIs and certain vaginal conditions can increase HIV transmission risk by up to 8 times. Genital ulcer diseases like herpes and syphilis create open sores that serve as direct entry points for the virus.
How STIs increase HIV vulnerability
STIs increase viral shedding in genital secretions and recruit immune cells that are targets for HIV. When someone with HIV also has an STI, their genital secretions contain higher concentrations of the virus. For the HIV-negative partner, STIs compromise the protective barriers of genital tissues through inflammation and ulceration, making it easier for HIV to establish infection.
Common bacterial infections like gonorrhea, chlamydia, and trichomoniasis all increase HIV transmission risk. Viral infections including herpes simplex virus and human papillomavirus also elevate transmission probability. Getting tested and treated for STIs represents a critical component of HIV prevention.

Awareness levels about HIV transmission vary widely across different communities and cultural contexts. Limited access to comprehensive sex education, language barriers, and cultural taboos around discussing sex can prevent people from obtaining accurate information about transmission risks and prevention methods. Stigma surrounding HIV also discourages testing and disclosure, which can inadvertently increase transmission.
Local practices and their impact
Certain cultural practices can increase HIV vulnerability. Cross-generational relationships with large age differences between partners have been linked to higher HIV transmission rates, as have specific sexual practices that can cause tissue damage or inflammation. Cultural prohibitions around discussing sexual health or using protection methods like condoms can limit people’s ability to negotiate safer sex.
Gender dynamics within relationships also play a crucial role. In some cultural contexts, women may have limited power to negotiate condom use or insist on partner testing. Economic factors intersect with culture as well-poverty can drive transactional sex and limit access to healthcare services, while lack of education restricts health literacy about HIV transmission and prevention.
The role of health literacy and access
Understanding how HIV is transmitted and prevented requires access to accurate health information. Communities that face barriers to healthcare due to language differences, lack of insurance, or cultural disconnection from medical systems often have higher HIV transmission rates. Misinformation and outdated beliefs about HIV can persist when people lack access to current, evidence-based health education.
Effective HIV prevention requires culturally appropriate interventions that respect local contexts while providing scientifically accurate information. Programs that incorporate cultural understanding while teaching practical risk reduction skills have shown greater success in reducing transmission rates than those that ignore cultural factors.
